  5. A statement by you, made under penalty of perjury, that the above information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner’s behalf.

  6. Your physical or electronic signature.

Counter-Notification

If you believe that your material was removed or disabled by mistake or misidentification, you may submit a counter-notification. Your counter-notification must include:

  1. Your name, address, phone number, and email address.

  2. A description of the content that was removed and the location where it previously appeared.

  3. A statement under penalty of perjury that you believe the content was removed or disabled as a result of a mistake or misidentification.

  4. A statement that you consent to the jurisdiction of the federal district court in your location (or in Windhoek, Namibia if you are outside the U.S.), and that you will accept service of process from the person who submitted the original DMCA notice.

  5. Your physical or electronic signature.

Upon receiving a valid counter-notification, we may reinstate the removed content within 10 to 14 business days unless the original complainant files a court action.
